Baby is not taking formula milk

My baby is 5 month old and he is underweight.. He used to take formula milk once from birth and took it for 2 month.. After that suddenly he stopped now I am trying my best to feed him with formula milk but he is not taking.. He is totally into breastfeed.. I am really concerned whether my feed is sufficient or what could be the reason for suddenly stopping formula milk..
